HelpAge Tanzania (HAT) is a national non-governmental organisation registered in Tanzania Mainland under the Non-Governmental Organisations Act of 2022 and compliant in Zanzibar under the Business Entities Registration Act No. 12 of 2012.
HAT works to ensure that older people, people with disabilities, and other people with specific needs (PSNs) are protected, included, and able to live with dignity. In Kigoma Region, HAT implements inclusive humanitarian programmes in Nduta and Nyarugusu refugee camps and host communities, focusing on protection, health, care, psychosocial wellbeing, and inclusion of the most vulnerable.
The Integrated Care Nurse (ICN) is responsible for delivering person-centred, holistic, and integrated care services to older people, persons with disabilities, people with serious or chronic medical conditions, unaccompanied and separated children, and other marginalized People with Specific Needs (PSNs) in Nduta and Nyarugusu refugee camps. The role focuses on improving quality of life, functional ability, comfort, dignity, and psychosocial wellbeing through preventive, curative, rehabilitative, supportive, and end-of-life care, as appropriate, within a humanitarian context. The ICN works closely with health facilities, protection teams, community structures, and humanitarian partners to ensure coordinated and continuous care.
The position is based in Kigoma Region, with primary responsibility for service delivery in Nduta and Nyarugusu refugee camps. The post holder will:
Provide direct integrated nursing care to PSNs
Support continuity of care between community, health facilities, and home-based settings
Contribute to multidisciplinary case management and referrals
Support caregivers, families, and community-based care structures
Contribute to reporting, learning, and quality improvement
Provide comprehensive nursing care for older people, people with disabilities, and other PSNs, including those with chronic illnesses, disabilities, serious medical conditions, and terminal illnesses.
Conduct holistic assessments covering physical, psychological, social, and functional needs.
Provide symptom management (e.g. pain, breathlessness, wounds, mobility challenges) in line with clinical guidelines.
Support activities of daily living, comfort care, and dignity-focused interventions.
Identify early deterioration and ensure timely referral to appropriate health services.
Provide home-based and community-based nursing care to PSNs who have limited mobility or access to health facilities.
Support caregivers and families with practical skills, counselling, and education on care practices.
Promote continuity of care following discharge from health facilities.
Work with community volunteers and outreach workers to identify and follow up PSNs.
Provide basic psychosocial support to PSNs and their caregivers, including emotional support and counselling.
Support individuals and families facing serious illness, disability, loss, or end-of-life situations.
Promote dignity, respect, cultural sensitivity, and informed decision-making in care.
Participate in multidisciplinary case management for PSNs, working closely with protection, health, nutrition, child protection, and disability services.
Facilitate referrals to health facilities, rehabilitation services, mental health and psychosocial support (MHPSS), protection services, and social assistance.
Liaise with UN agencies, health partners, and camp health facilities to ensure coordinated care.
Provide health education on chronic disease management, disability care, nutrition, hygiene, and infection prevention.
Support preventive care including early identification of complications and risk factors.
Promote self-care and caregiver resilience within the refugee context.
Maintain accurate and confidential patient records and case documentation.
Contribute to programme reporting, referrals tracking, and follow-up data.
Support monitoring of outcomes related to quality of life, functional ability, and wellbeing.
Participate in learning and quality improvement initiatives.
Ensure care is delivered in line with safeguarding, protection, and humanitarian principles.
Identify and report protection concerns, abuse, neglect, or exploitation through established referral mechanisms.
Ensure informed consent, confidentiality, and respectful care at all times.

Diploma or bachelor’s degree in nursing (Registered Nurse)
Valid nursing registration with relevant professional council
At least 3 years’ experience in clinical nursing practice
Experience working with vulnerable populations, including older people, people with disabilities, or people with chronic or serious illnesses
Experience in humanitarian, refugee, or low-resource settings preferred
Strong skills in holistic and person-centred nursing care
Knowledge of chronic disease management, disability care, and basic palliative care principles
Ability to provide compassionate psychosocial support
Good communication and teamwork skills
Ability to work in challenging humanitarian environments
Training or experience in palliative care, integrated care, or community/home-based care
Experience working in refugee camps or emergency settings
Knowledge of protection, safeguarding, and referral systems
Ability to speak Kiswahili; Kirundi is an added advantage
