Dear Sir/Madam 

Americares Tanzania is seeking qualified Health Insurance Provider to provide their best price/quality quote for the item(s) below.  If interested, the quotation should be submitted on company letterhead and signed.  Please refer to the Terms and Conditions (below) in preparing your quotation.  Please acknowledge receipt of this request and indicate an intention to quote by signing, stamping, and sending back to us this RFP.

Quote Validity Requirements

Specifications: 

Vendor shall be responsible for plan administration (enrollment, id cards, orientation, orientation materials, and responsive emergency contact channels), responsible for all claims incurred from the effective date through termination, provide quarterly reports on paid versus paid premiums, provide 3 references of terminated and active contracts within the last 2 years that are approximately our team size.

Vendor Eligibility Requirements:

This RFQ is open to all Insurance companies providing health Insurance Services, licensed in Tanzania, capable of providing and delivering the products, and with a solid record of integrity and business ethics.   Note: Americares Tanzania will not award a contract to any firm that is debarred, suspended, or proposed for debarment, or who proposes to do business with firms or firms’ principals who are debarred, suspended, or proposed for debarment, in the performance of the requirement of this activity.  

Payment Terms: 

• 100% payment 30 days after delivery and receipt of invoice. 

Quotation must include the following: 

• Please include in your proposal hospitals that provide one of the very best health care service providers.
• Quote Details: Quote shall be for a period of 12 months and marked with PR# above; Detailed description of the quoted items; Information on delivery cost, method, date; Quotation in Tsh. Inclusive of tax, excise and other duties or taxes; Company Details: Copy of valid Company Trade license, Tax certificate (VAT Registration).    
• Coverage for vision lenses and COVID 19 pandemic.

Evaluation criteria: 

Offers will be evaluated based on their responsiveness to the specification, qualifications, and experience of the vendor, price, after-sale service, delivery and installation time, and related criteria.  Americares Tanzania will select the offer featuring the best coverages that meet our team’s needs.

Submission of Quotes 

Submission Deadline: 

• The quote must be submitted to Americares no later than March 7^th, 2022, at 4:00pm. 
• Quotations submitted after the deadline will not be considered.

Terms and Conditions

1. Issuance of this RFQ does not constitute an award commitment on the part of the Americares Tanzania, nor does it commit Americares Tanzania to pay for costs incurred in the preparation and submission of a bid.
2. Attachments to this RFQ are considered integral to all requirements.
3. Americares Tanzania may contact bidders to confirm the contact person, address, bid amount and to confirm that the bid was submitted for this solicitation.
4. False Statements in the Bid: Bidders must provide full, accurate, and complete information as required by this solicitation and its attachments.
5. Mistakes In Proposals: If a mistake in a proposal is suspected or alleged, the proposal may be corrected or withdrawn during any negotiations that are held. If negotiations are not held, or if best and final offers have been received, the Respondent may be permitted to correct a mistake in the proposal and the intended correct offer may be considered based on the condition that the Respondent submits written evidence, which clearly and convincingly demonstrates both the existing offer and such correction would not be contrary to the fair and equal treatment of other Respondents.
6. Conflict of Interest Disclosure: Bidders must provide disclosure of any past, present and/or future relationships with any parties associated with the issuance, review or management of this solicitation and anticipated award.  Failure to provide full and open disclosure may result in Americares Tanzania having to re-evaluate the selection of a potential Bidder.
7. Right to Select/Reject: Americares Tanzania reserves the right to select and negotiate with those firms it determines, in its sole discretion, to be qualified for competitive quotes and to terminate negotiations without incurring any liability.  Americares Tanzania also reserves the right to reject any or all quotes received without explanation when considered to be in the best interest of the foundation and/or the people it serves. 
8. Reserved Rights: RFQ responses become the property of Americares Tanzania. Americares Tanzania reserves the right in its sole discretion:

1. To disqualify any offer based on Bidder’s failure to follow solicitation instructions.
2. To waive any deviations by Bidder from the requirements of this solicitation that in Americares Tanzania’s opinion are considered not to be material defects requiring rejection or disqualification; or where such a waiver will promote increased competition.

• To extend the time for submission of all RFQ responses.

1. To terminate or modify the RFQ process at any time and re-issue the RFQ.
2. To issue an award based on the initial evaluation of offers without discussion.

Americares Code of Conduct & Ethics Policy: In accordance with the Americares Code of Conduct and Ethics Policy, Americares requires full and open disclosure when dealing with procurement. As such, Americares employees must avoid any conflict of interest or the appearance of a conflict of interest. Americares employees must at all times provide full disclosure of their actions or relationships with prospective vendors, contractors, or consultants.  Americares employees shall not solicit, request, accept, or agree to accept any gift from a vendor or prospective vendor.
